Job Summary:
We are looking for a dedicated Registered Nurse (RN) to join our Home Health team. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Duties and Responsibilities:
- Deliver high-quality patient care in the comfort of patients' homes
- Develop and implement individualized care plans that meet patients' unique needs
- Work collaboratively with patients, families, and healthcare teams to achieve optimal patient outcomes
- Maintain accurate records and communicate effectively with colleagues and stakeholders
Qualifications:
- Current Illinois RN license
- One year of medical surgical experience
- Strong communication and interpersonal skills
- Able to lift up to 35 pounds without assistance
- Ability to work weekends, holidays, and varying shifts
