This exciting opportunity allows a qualified Family Practice MD in Illinois to fill a crucial gap in care at a local healthcare facility.
The provider will work in a home health setting, visiting homes and nursing facilities, to provide comprehensive eye care to patients.
Responsibilities include conducting eye exams, diagnosing and treating ocular diseases, prescribing and fitting eyeglasses, and advising patients and families on follow-up care.
This position provides a unique chance for an experienced optometrist to deliver high-quality eye care to patients in need, while utilizing the Nextgen electronic health record system.

Flexible, Monday-Friday, 8:30 AM - 4:00 PM Responsibilities and Duties:
- Conduct comprehensive eye examinations
- Diagnose and treat ocular diseases
- Presume, fit, and adjust eyeglasses
- Advise patients, families, and facilities on follow-up care
- Travel to homes and nursing facilities
- Maintain accurate and detailed patient records using Nextgen software
- Ensure patient satisfaction and high-quality care
- Adhere to home health policies and procedures
- Collaborate with other healthcare professionals as needed
- Stay updated on current optometry practices and techniques
Requirements and Qualifications:
- Must be licensed in the state
- Comfortable with ocular disease management
- Experience with home health or mobile optometry preferred
- No prescribing required
- Ability to travel to various locations within the specified areas
- Flexible scheduling within the given timeframe
