Home Health Care Specialist

Job Description:
We are seeking a skilled Registered Nurse (RN) to join our Home Health team. The ideal candidate will have experience in medical surgical settings and possess strong communication and interpersonal skills.
The RN will provide high-quality care to homebound patients, working collaboratively with multidisciplinary teams to develop and implement comprehensive care plans.
Responsibilities:
- Assess patient needs and develop individualized care plans
- Provide skilled nursing care to patients in their homes
- Collaborate with patients, families, and healthcare teams to achieve optimal patient outcomes
- Maintain accurate records and communicate effectively with colleagues and stakeholders
Requirements:
- Current Illinois RN license
- One year of medical surgical experience
- Strong communication and interpersonal skills
- Able to lift up to 35 pounds without assistance
- Ability to work weekends, holidays, and varying shifts
