LPN (Licensed Practical Nurse) - House Manager
Responsible for the health and welfare of the people supported in the medical residential group home and providing a family-type home atmosphere. Manages all activities including upkeep of home and food budgeting. LPN staff at Sertoma have an exceptional nurse to patient ratio of 3:1 and are assigned a full-time Direct Support Professional as an assistant.
Shift: 12-hour, day shift (8 am - 8 pm)
- Reports to: Agency RN for clinical and Residential Directors and Residential Coordinators for residential
- Supervises: Three people supported and a full-time assistant.
- Works with: Individuals we serve, human resources, case managers, clinic staff, residential staff, employment staff
- External Relationships: TNDIDD, Managed Care Organizations, family/guardians/conservators, employers, community partners, Independent Support Coordinators, Support Coordinators (ECF), physicians, pharmacy
- Qualifications: Current unencumbered LPN license
- Work Experience: Previous experience with ID/DD population preferred but not required. Time management skills and ability to multi-task are beneficial in this position
- Physical Requirements: Sight and hearing, ability to lift/assist people supported and to intervene during behavioral episodes in a manor in keeping with company policies
Responsibilities:
- Ensure health and safety of people supported by ensuring adequate exercise, providing assigned nursing services, maintaining up to date postings of medical/emergency information, ensuring home is hazard free, participating routinely in training for fire evacuation, maintaining clean/sanitary environment and reporting any maintenance needs promptly.
- Ensure a family atmosphere through interaction, counseling, outings, conversation, etc.
- Meet the needs of the people supported through training and maintaining a clean home, meal preparation, assisting with personal hygiene, and shopping.
- Conduct and participate in monthly house meetings.
- Encourage people supported to follow dietary menus. Post all menus/special diets for easy access for other staff.
- Plan and follow through with regularly scheduled recreational activities at the house.
- Maintain house records in an orderly and up to date manner (i.e., fire drills, med requests, on-site person supported records, licensure, ISP, etc.)
- Work regular schedule, participate as a team player, offer to cover, or swap shifts when time off is needed. Sertoma nurses are required to cover shifts with existing nursing staff.
- Distribute medications and other medical treatments as prescribed. Maintain daily counts and reorder medications as necessary according to agency policy and procedures. Report medication errors when discovered.
- Attend circle of support meetings, monthly staff meetings and in-service meetings, ISP meetings. Provide input to the RN, case manager and residential department on all plans.
- Maintain the vehicle logs to include regular cleaning, monthly mileage form, gas receipts etc. Promptly report any vehicle problems to the maintenance department via work order or phone call to maintenance department.
- Submit monthly paperwork to the residential department according to schedule as assigned.
- Familiarize self with licensure, fire marshal and quality assurance standards and always maintain group home within these standards. Meet with residential director or residential coordinator regarding any issues.
- Communicate with the appropriate staff immediately regarding any concerns, problems, or outside issues from external relationships.
- Complete and document nursing notes daily.

- You will perform any other duties as assigned by the Registered Nurse, Chief Operating Officer, Sr. Director of Residential Services, Director of Residential Services, or Residential Coordinator.

- Maintain ongoing communication with staff as appropriate, keeping staff informed of all pertinent medical information.
- Assist RN in ordering medication, checking in medication when delivered and distributing medication to the appropriate staff.
- Assist RN in reviewing monthly Medication Administration Records (MARs) prior to
- distribution to the residential sites for accuracy.
- Monitor documentation follow up from medical providers (labs, x-ray/scan results, office notes, etc.) and enter into TIMAS
- Assist in counseling individuals as needed.
- Monitor blood pressure or blood glucose readings of designated individuals as needed per MD orders and document the readings in TIMAS. Documentation will be provided to the physician during scheduled appointments.
- Assist in maintaining a medication history on each individual supported.
- Monitor weights as recommended by the individual's physician, documenting the results in TIMAS.
- Provide and monitor sick slips as appropriate, for individuals who take medication for constipation, sleeping difficulties, menses cycle, incontinence, PRN medication doses, or general illness, etc.
- Serve in the absence of the RN in coordinating the activities of the Clinic.
- Participate in rotating weekend "on call" responsibilities with the RN.
- Adhere to an acceptable level of professionalism to ensure the maintenance of
- confidentiality and discretion necessary in the maintenance medical records.
- As a representative of Sertoma, maintain professionalism in all interactions with
- outside entities.
- Contribute to the "effort" of the Center itself by continually being alert to new concepts which could improve or enhance either your job individually or the collective functions of the Center.
- Observe all Center routine regarding work schedules, presence during working hours which conform to those of the Center, designated periods of break time, lunch hour, etc.
- Perform all work assignments and activities necessary to the professional operations of the LPN position in an acceptable manner and require minimal supervision from the RN.
- Perform any other duties as assigned by the RN or member of senior leadership.
