Home Health Case Manager

We are seeking a skilled Home Health Case Manager to join our team. As a Home Health Case Manager, you will be responsible for providing high-quality nursing care to patients in the comfort of their own homes. This role requires excellent communication and interpersonal skills, as well as the ability to work independently and make decisions in a fast-paced environment.
The ideal candidate will have a strong background in medical-surgical experience and a current Illinois RN license. You will also need to be able to lift up to 35 pounds without assistance and have good documentation skills.
In this role, you will be responsible for assessing patients' needs, developing individualized care plans, and coordinating with other healthcare professionals to ensure seamless patient care. You will also be expected to document all patient interactions and outcomes, as well as participate in ongoing education and training to stay up-to-date on the latest best practices.
