Home Health RN Case Manager Position

About the Position:
We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team as a Case Manager. In this role, you will have the opportunity to work closely with patients and families in their homes, providing personalized care and support to help them achieve their maximum potential.
Responsibilities:
- Conduct thorough assessments of patients' physical and emotional needs, developing individualized plans of care that address their unique requirements
- Deliver high-quality nursing care, prioritizing patient safety and comfort while promoting independence and self-care
- Collaborate with healthcare providers and other stakeholders to coordinate care and ensure smooth transitions between settings
- Use electronic health records and other systems to document patient information accurately and efficiently
- Participate in quality improvement initiatives, contributing to ongoing efforts to enhance patient outcomes and satisfaction
Qualifications:
- Education and Experience:
- Associate's or Bachelor's degree in Nursing
- Minimum one year of medical-surgical experience in an acute or subacute care setting
- Active RN licensure in Illinois
