Community Health Specialist

Job Title: Community Care Clinician RN

This role involves delivering medical care in community settings where patients reside, including homes, shelters, offices, public places, or on the street. The clinician is responsible for facilitating virtual provider visits and following medical orders. Responsibilities include performing clinical skills and tasks within their scope, such as IV insertion, medication administration, EKGs, phlebotomy, and others as permitted by local regulations.

Key Responsibilities:

• The clinician will play a vital role in delivering high-quality patient care by ensuring they are well-equipped, operationally ready, certified, trained, and effective.

• The clinician is pivotal in connecting patients to healthcare services, including medical providers and access to all available services.

• The clinician will be responsible for various visit types, including acute illness facilitated provider visits, longitudinal care team facilitated provider visits, outreach visits, specimen collection visits, MIC only follow-up visits, and other related duties.

Requirements:

• 4+ Years of Work Experience

• Bachelor's Degree
